Person-centred care makes a real difference to people affected by cancer. The Transforming Care After Treatment (TCAT) programme set out to ensure everyone with cancer receives this kind of care, both in health and social care.
And it's working. Since the programme launched in 2013, 25 projects have been testing new models of care. We want to share their successes and spread the learning across Scotland and beyond.
